Create a Coordinated Entry Event

Follow these instructions when:
- A Problem-Solving/Diversion/Rapid Resolution conversation has taken place,
- When making a referral to a Housing Navigator,
- Or when making a referral to a Housing Program/Provider. 

Do not create an event unless it is part of your HMIS workflow.

Problem-Solving, Diversion, or Rapid Resolution Event

Target Audience: Assessors, Diversion Case Managers

  1. Select your Coordinated Entry Diversion provider to enter data as.
  2. Open the client's record and go to the Entry/Exit tab. Add an interim review to their Coordinated Entry Diversion row.
    1. If assisting a household, make sure to include every household member by checking the box next to their name.
    2. Choose the Update interim review type.
  3. In the interim assessment, scroll down to the Coordinated Entry Event sub-assessment. Click on the Add button.
  4. Fill out these fields in the Event pop-up window:
    1. Start Date: The date that the event took place.
    2. Date of Event: Same as the Start Date.
    3. Event: Select Problem Solving/Diversion/Rapid Resolution intervention or service result.
    4. Problem Solving/Diversion/Rapid Resolution intervention or service result: If you know whether the client was housed or re-housed in a safe alternative, then select a Yes or No answer at this time. If you do not know yet, you can update this event later.
  5. Click on the Save button at the bottom of the Event pop-up window.
  6. Close the interim assessment by clicking on the Exit button.

Referral to a Housing Navigation Program

Target Audience: Priority List Managers

  1. Select your Priority List provider to enter data as.
  2. Open the client's record and go to the Entry/Exit tab. Add an interim review to their Coordinated Entry Assessment row.
    1. If referring a household, make sure to include every household member by checking the box next to their name.
    2. Choose the Update interim review type.
  3. In the interim assessment, scroll down to the Coordinated Entry Event sub-assessment. Click on the Add button.
  4. Fill out these fields in the Event pop-up window:
    1. Start Date: The date that the referral was made.
    2. Date of Event: Same as the Start Date.
    3. Event: Select Referral to Housing Navigation project or services.
  5. Click on the Save button at the bottom of the Event pop-up window.
  6. Close the interim assessment by clicking on the Exit button.

HMIS will not automatically send a notification when a referral is made. Priority List Managers will need to manually notify the navigator through email or another communication channel.

Referral to a Housing Program

Target Audience: Priority List Managers

  1. Select your Priority List provider to enter data as.
  2. Open the client's record and go to the Entry/Exit tab. Add an interim review to their Coordinated Entry Assessment row.
    1. If referring a household, make sure to include every household member by checking the box next to their name.
    2. Choose the Update interim review type.
  3. In the interim assessment, scroll down to the Coordinated Entry Event sub-assessment. Click on the Add button.
  4. Fill out these fields in the Event pop-up window:
    1. Start Date: The date that the referral was made.
    2. Date of Event: Same as the Start Date.
    3. Event: Select the option that matches the type of housing the client is being referred to: Transitional Housing (TH), Rapid Re-Housing (RRH), Joint TH-RRH, Permanent Supporting Housing (PSH), or other Permanent Housing (PH S or PH HO).
    4. Location of Crisis Housing or Permanent Housing Referral: Click on Lookup, then select the housing provider that the client is being referred to.
    5. Housing Provider Contact Details: Enter the housing provider contact’s information.
  5. Click on the Save button at the bottom of the Event pop-up window.
  6. Close the interim assessment by clicking on the Exit button.
*HMIS  will not automatically send a notification when a referral is made. Priority List Managers will need to manually notify the housing provider through email or another communication channel. Housing providers can check if they have pending referrals by running the 258 CE Housing Referrals report.
