Hennepin: CoC Care Coordination in HMIS

What is Care Coordination in HMIS?

The Care Coordination assessment, combined with the other information on the Summary tab in HMIS, brings together information from a variety of places in HMIS to show an up-to-date "snapshot" of a client's HMIS record. This can help HMIS users across the continuum who work with an individual to see and update critical information that can be used to connect someone with needed resources.

Information on the Care Coordination Assessment is organized into the following sections:

  1. Care Coordination Notes
  2. Key Contacts
  3. Case Management Interest
  4. Contact Information
  5. Reference Only Information

Most of the questions that appear on this assessment also appear on other assessments within the Hennepin CoC Homeless Response System. Information is shared between assessments.

Who can use the Care Coordination Assessment

Available for projects operating in Hennepin County only! The Care Coordination Assessment is currently available to all projects in HMIS except housing projects (Rapid Re-Housing, Permanent Supportive Housing, Permanent Housing with Services, and Transitional Housing) and homelessness prevention projects. Hennepin CoC will evaluate the use of this assessment and whether it should be made available to a different set of projects in the future. 

How to access the Care Coordination Assessment in HMIS

  1. Enter EDA (Enter Data As) mode to the correct project provider. 
  2. Navigate to the client’s record in ClientPoint. 
  3. In the backdate pop-up, select Current System Date to see the most up-to-date information on the Care Coordination assessment. 
  4. Click on the Summary tab of the client record.
  5. At the top of the Summary tab, you have access to 5 “Client Summary" dashlets that display information from other tabs of the client record. These may be different than the dashlets you are used to seeing on the Summary tab. These 5 dashlets have been displayed to assist you with Care Coordination efforts.
    1. Houseolds
    2. Entry/Exits
    3. Shelter Stays
    4. Services
    5. Case Managers

  6. At the bottom of this tab, you’ll see the Care Coordination in Hennepin CoC assessment. 
  7. Carefully review all blue, red, and green text provided to assist you in navigating and entering information into this assessment. 

Indicating interest in Housing Case Management in HMIS

HMIS users at Hennepin County operating homelessness projects can add residents to the Housing-focused Case Management Interest List. If you are talking with, encountering, or working with a client who is interested in housing-focused case management, you can indicate that in HMIS on the Care Coordination Assessment. You do not need to complete the entire Care Coordination Assessment in order to indicate someone’s interest in Case Management.
