05. Enrollment: Barriers/Disabilities Assessment

This article will provide instructions for completing the Barriers assessment, also known as Disabilities or Special Needs, during the program enrollment workflow.

More About This

Most projects are required to collect information about Barriers, also known as Disabilities or Special Needs, present for the client at the time of enrollment. Your answer to the Disabling Condition question on the previous Universal Data Assessment form must match the selected barriers on this Barriers form.

For example, if you selected No for the Disabling Condition question previously, you should have all the conditions marked No on this page as well.  

For all conditions except HIV/AIDS and Developmental Disability, the follow-up field Condition is Indefinite will appear:

  • If the disability is present and expected to continue indefinitely, it should be marked Yes.

  • It is possible for a client to have a disability and also answer No to the Condition is Indefinite question, if the condition is not expected to be long-term or substantially impair the client's ability to live independently.

  • Note that if it is marked No, the client may not qualify for programs meant for people with severe and persistent disabilities, and the barrier may not register as a disabling condition.

Details

Use this form to identify whether or not a client is experiencing each barrier in the list. The client's last assessment data will be displayed by default.

💥NOTE: Users are required to record a status for all Barrier rows on this sub-assessment!

  1. Answer the initial questions as appropriate:
    1. The Identified Date field should auto-fill with the same date as the start of your program enrollment.
    2. The Screen field should auto-populate to HMIS Barriers. You can leave this as-is.
    3. The Disabling Condition question appears as greyed-out, or as read-only. This is provided here for your reference, as you previously answered this question on the Universal Data Assessment:

  1. Next, scroll down to the sub-assessment, and click the top check box above the checkbox column. This will set all the rows below to an answer of "No" for the Barriers Present? column:

  1. If the client does have a Disabling Condition present, scroll down to locate the applicable Barrier and select Yes in the drop-down menu under the Barrier Present? column. (If the client does not have a Disabling Condition, you can click the Save& Close button at the bottom of the page to move on.)

  1. Use the horizontal scroll bar to scroll to the right, and fill out the drop-down field in the Condition is Indefinite. (This dropdown will appear for all barriers except HIV/AIDS and Developmental Disability):

  1. You can add summary information in the optional Explanation column (this is not required):

  1. The checkbox for each row in the Previous Barriers Detail column can be clicked to view information about the auto-filled previous records for your client:

  1. Click Save & Close in the bottom right corner of the page to move on to the next step of the workflow:
