- Make sure you have selected the correct Workgroup and Organization!
- Check out the Choosing your Workgroup & Organization for more information on workgroups & organizations.
Pull up the Client Dashboard for the client you are hoping to add the Care Coordination note for:
In the left hand menu, click on the Enrollment and Services Folder and select Common Workflow forms:
Select Care Coordination Notes:
Record an Information Date and Notes within the form:
- Don't forget to click Save.
