More about This:

This form is part of the Hennepin Singles Coordinated Entry Workflow and Hennepin Families Coordination Entry Workflow. It will display at Project Start and Update for the head of household. 

Details:

  1. Make sure you have selected the correct Workgroup and Organization!
    1. Check out the Choosing your Workgroup & Organization for more information on workgroups & organizations.
  2. Pull up the Client Dashboard for the client you are hoping to add the Care Coordination note for:

  3. In the left hand menu, click on the Enrollment and Services Folder and select Common Workflow forms:

  4. Select Care Coordination Notes:

  5. Record an Information Date and Notes within the form:

  6. Don't forget to click Save.
