Hennepin CoC: Care Coordination in HMIS

Overview

The Care Coordination tools in HMIS bring together information from a variety of places in HMIS to

show an up-to-date "snapshot" of a client's HMIS record. This helps HMIS users see and update

critical information to connect someone with needed resources.


Forms available in the Care Coordination menu folder are organized into the following sections:

  • Care Coordination Review: A read only summary of the information in the forms below that can be printed
  • Shelter Placement: Includes Case Management Interest and Avivo Village Interest
  • Contact Preferences
  • Key Contacts: Additional workers associated with the client who aren’t necessarily housing case managers
  • Alternative Contacts
  • Case Managers: Lists all current and previously associated enrollers and case managers associated with a client
  • Current Living Situation
  • Vital Documents: A place to document if a client has their ID, Social Security Card, or Birth Certificate(for themselves or their dependents)
  • Family CE Assessments Appointments: Check here to see if a family has or has recently had an appointment for a CE assessment
  • CE Event: Find a list of all current and previous CE Referrals and Diversion encounters

Most forms listed above appear in multiple workflows within the Hennepin CoC Homeless Response System. Information is shared between assessments.


Who can Access the Care Coordination Folder

The Care Coordination Folder is available for organizations operating in Hennepin Continuum of Care (CoC). If your organization also works in another CoC, you will see this folder even if you are entering data for the other CoC.

How to access the Care Coordination Menu in HMIS

  1. Navigate to the client’s record.
  2. From the Client Workspace, navigate to the menus on the left.
  3. In the favorites section, notice the Care Coordination Review (Read Only) form.

  1. Further down the menu options, you’ll see the Care Coordination menu folder. Review or update information as needed on any available form.
