High Priority Homeless (HPH) Eligibility Confirmation

MN Housing LTH/HPH units are required to be filled by Coordinated Entry referrals. You can pull documentation of the referral from HMIS for your client's file.
Note: The client must be enrolled in your LTH/HPH project before you are able to generate this form.
How to Run the High Priority Homeless (HPH) Eligibility Confirmation Report
This report is located in the Client Workspace.
- Select the Client workspace.
- Select the Client-Specific Reports Folder and select HPH Eligibility Confirmation.
- Use the dropdown to select the client’s LTH/HPH project enrollment.
- Click the Report button located in the bottom-right corner.
- When the report is finished running, it can be found in the Report Workspace.
- Under My Saved Reports.
- Click on the blue date to open the report.
How to Save a Copy of the HPH Eligibility Confirmation Report
- To save a copy, click the Save icon.
- Select the PDF option.
How To Print the HPH Confirmation Report
- Select Print Icon.
- A Print pop up will appear, click Print.
Note: This form contains client names. Do not email this report or store it in an unsecure location.
Troubleshooting
Who currently has access to run this report?
HMIS users with 'active' rights to the organization that created the LTH/HPH enrollment can generate the report, as well as the organization that made the referral.
What if my project does not show up in the report picklist?
Confirm that you've enrolled the clients in you LTH/HPH project.
Then confirm the CE Event field "Resulting Enrollment" lists the project as well.
If one or both are missing, then your project will not display in the report prompt picklist.
