When is City Where Resides collected in HMIS?

  • You must collect this data element for Head of Households enrolled in FHPAP and HSWH projects during:
    • Project Enrollment
    • Enrollment Updates
    • Enrollment Exit

What HMIS form does City Where Resides appear on?

  • City Where Resides is collected on the Universal Data Assessment (UDA) form during program enrollment, update, or exit workflows. It will display after you complete the County Where Resides field.

Step by Step for Correcting a Missing City Where Resides

If City Where Resides is missing from your cilent's enrollment: Follow the steps below to correct it

  1. Go to the Client Workspace. Search for your client and select their name from the search results.
  2. Navigate to your Client's Active Enrollments. 
  3. Find your client's FHPAP or HSWH enrollment. Click the Action button next to it.
  4. Choose Edit Project Entry Workflow from the menu that appears.
  5. Click through the workflow until you reach the Universal Data Assessment form.
  6. Complete the City Where Resides field. It is located below the County Where Resides field.
  7. Click Save at the bottom right corner on your screen.
  8. Continue through the remaining workflow forms. Click Finish to finalize the Entry Workflow.

Important: If you leave the page before clicking Finish, it will cause data problems.
