Recording RHY Aftercare

Overview

Aftercare Services are extra help provided to youth after they have been exited from your RHY program. These services support them and keep the connection going after they leave your program, and are in accordance with the RHY Rule [45 CFR §1351].

When to Record Aftercare Services in HMIS

You should record Aftercare Services from the day youth are exited from your RHY program, up to 90 days after (3 months).

Each client who has been exited from your RHY program must have at least one Aftercare Service recorded. This Service record must say one of the following:

  • Yes – aftercare was provided
  • No – aftercare was not provided
  • Client refused Aftercare Services

If aftercare was provided, use the date the service was first given as the information date

If no aftercare was provided, set the information date as the date the person exited the project, and choose “no” for the aftercare question.

How to Record Aftercare Services in HMIS

  1. Go to the Client Workspace. Search for the client and select their name from the search results.
  2. Navigate your client's Exited Enrollments
  3. Find the exited RHY enrollment. Click the Action button next to it.
  4. Choose RHY Aftercare from the menu that appears. This option only shows if the client has an exited RHY enrollment.
  5. On the Create Aftercare Assessment form, select "Yes."
  6. Fill in the Aftercare was Provided field. If you select "Yes," you must also fill in how the aftercare was provided. Click Save.
  7. Repeat Step 6 for each household member.

    Note: Not all household members will need this. It will only show for the Head of Household or adults.

  8. Click Finish. This finalizethe Aftercare Workflow.

    Important: If you leave the page before clicking Finish, it may cause data problems.

Note: Aftercare can go on longer than 90 days, but you only need to record the first service after exit. You do not need to start a new project to track services after the 90-day period.
