Hennepin CoC: Goals for Case Management Programs

Overview
This article briefly reviews the process to create, edit, update, and resolve goals in HMIS.
The Goals Section is available for all users and all clients. Below are brief instructions for the suggested use for Case Management programs.

Navigating to the Goals Form
Goals are located in the Client Workspace in the Profile menu folder.
On the Goals screen, users can select any of the following actions:
- Add New Goal
- Edit or update existing goals
You can also use the Client Goal Report to get a summary of information about your client’s goals and outcomes.
Adding a New Goal
Status: Defaults to Open (close when complete or when the client exits your program)
Goal Case Manager: Defaults to the user creating. Another Case Manager can be selected if needed
Goal Type: Short Term or Long Term. This is optional and can be left blank
Goal Group: This replaces the term “Goal Classification” and is the larger category of goals
Goal: Displays options based on the Goal Group selected above
Goal Description: Add notes here to further describe the goal if needed
Task: Optional. You can create a task with or without a notification that is added to the client’s calendar. This can be shared systemwide.
Goal Date: Record date goal is set
Target Date: Record date goal is expected to be complete
Enrollment: Select the Case Management Enrollment for your client
Explanation: An optional area for more notes related to the client goal
Restriction: Set goal to be visible w/in your organization only or for all organizations
Updating Goals and Recording Outcomes
On the Goals page you will find a list of goals and a summary of their status. Click the Action Menu (…) to edit the goal or outcome, or record goal progress You can also record related case notes or create related services from this action menu.
From the Action Menu, you can also click on:
- Goal Progress: Make notes about how far along a goal is toward completion
- Case Notes: Add specific Goal case notes
- Services: Record Services provided related directly to a Goal
- Delete Goal
Edit Goal
To edit the goal or record an outcome, select Edit Goal from the action menu. When the goal is complete or the client has exited your program, update the following fields.
Status: Closed
Outcome: Attained or Discontinued. If the client exits prior to completion of the goal, select Discontinued.
Outcome Date: Record the date completed or the date the client exited
Explanation: Add any additional notes about the client’s progress or completion of the goal.
Click on Save.
