Completing the MN Triage Assessment

The MN Triage Assessment is a form within Coordinated Entry and Diversion workflows in HMIS. This article details instructions on how to complete the form.
The MN Triage Assessment form is completed for the Head of Household at enrollment and each time you complete an update assessment.
Follow these steps
Complete the Coordinated Entry Assessment section
- Assessment date: Add the date the assessment occurred.
- Assessment level: this should be pre-populated.
- Coordinated Entry: Housing Needs Assessment
- Diversion: Crisis Needs Assessment
- Assessment location: Add where the assessment took place
- Assessment Contact Type: this is how the assessment conducted. Select either over the phone, virtual or in person.
- Prioritization Status: Select either Placed on prioritization list or Not placed on prioritization list.
For Coordinated Entry enrollments, make sure to select Placed on Prioritization List for Prioritization Status.
The Coordinated Entry Assessment section is a HUD-required data elements that ensures clients are pulled appropriately into the CE-APR. This field does not actually determine whether or not a client is placed on the By-Name (Priority) List. Completing this enrollment process is what will get the client on the list.
Complete the Assessor Information
Select the assessor who completed this assessment using the search button. The assessor's contact details will be filled in automatically.
If the assessor is not found in the search, your priority list manager can add approved assessors upon request.
Complete the Current Living Situation
Information Date: Add the date the contact with the client occurred
Enrollment: This should pre-populate with the project you are creating an enrollment or update assessment for.
Current Living Situation: Select where the client is currently staying. This field has condition logic so if a temporary or permanent option is chosen, additional housing status information will appear that will need to be completed.
Location Details: Record details on where the client is staying.
When you are done, Click Save in the bottom right hand corner to continue through the workflow.
