Track Participation in a Housing Navigation Program (LTHSSF)

Workflow Overview

  1. Enroll the client in your Housing Navigation program when you begin working together.
  2. Update the client's information over time as you continue working together.
  3. Exit the client from your Housing Navigation program when you cease working together.
  4. Pull a report to review your program's service history.

Reminders:

  • Before entering data into a client's record, make sure to select your Housing Navigation provider from your "enter data as" list.
  • When you open a client's record and the Back Date Mode pop-up appears, select Use Current System Date. It is not necessary to enter a back date.
  • Data should be entered in the Entry/Exit tab. The Assessments tab should only be opened when you want to view a client's most recent assessment details.
  • If a client has worked with other homeless service programs before, data may already appear in their assessments. Review any existing assessment data and update information that is no longer accurate.

Program Enrollment

Enroll clients in your Housing Navigation program when you begin working together.

  1. Your region's Priority List Manager should send you an email when a referral is created for your program. That email will contain the client's ID number.
  2. Open the client's record and add a new entry for your Housing Navigation provider in the Entry/Exit tab.
    1. If assisting a household, make sure to include every household member by checking the box next to their name.
    2. Choose the HUD entry type if your provider features "HUD" in its name; choose the Basic entry type if it does not.
  3. Fill out the funder-specific "all inclusive" assessment as completely and accurately as possible. Not all DHS LTHSSF providers will have a funder assessment.
  4. Switch to your region's Coordinated Entry System (CES) assessment by choosing it from the Select an Assessment field. 
  5. Update the client's CES assessment as necessary.
  6. If you updated the client's CES assessment, switch to the LTHSSF Navigator Pilot assessment and record what updates you made.
  7. When you are finished, select Save & Exit at the bottom of the assessment window.

Update Information

Update clients' information over time as you continue working together.

  1. Open the client's record and add an interim update to their Housing Navigation row in the Entry/Exit tab.
    1. If assisting a household, make sure to include every household member by checking the box next to their name.
    2. Choose the Update interim review type.
  2. Fill out the funder-specific update assessment as completely and accurately as possible. Not all DHS LTHSSF providers will have a funder assessment.
  3. Switch to your region's Coordinated Entry System (CES) assessment by choosing it from the Select an Assessment field.
  4. Update the client's CES assessment as necessary.
  5. If you updated the client's CES assessment, switch to the LTHSSF Navigator Pilot assessment and record what updates you made.
  6. When you are finished, select Save & Exit at the bottom of the assessment window.

Program Exit

Exit clients from your Housing Navigation program when you cease working together.

  1. Open the client's record and close their open Housing Navigation entry in the Entry/Exit tab.
    1. If assisting a household, make sure to exit every household member by checking the box next to their name.
  2. Fill out the funder-specific exit assessment as completely and accurately as possible. Not all DHS LTHSSF providers will have a funder assessment.
  3. Switch to your region's Coordinated Entry System (CES) assessment by choosing it from the Select an Assessment field.
    1. Update the client's CES assessment as necessary.
  4. When you are finished, select Save & Exit at the bottom of the assessment window.
  5. If you updated the client's CES assessment, switch to the LTHSSF Navigator Pilot assessment and record what updates you made.

Review Service History

Pull a report to review your program's service history.

  • Run the 030 MN Core Homeless Programs report to view a list of the clients enrolled in your Housing Navigation program within a specified date range.
  • Run the 045 CES Navigator Projects report to view a listing of the changes recorded in the DHS LTHSSF assessment within a specified date range.
