Hennepin: Creating a Referral to Avivo's Pilot Project

This article details the step-by-step process for Hennepin Shelter Hotline users to create a referral for Doubled up Families to Avivo’s Hennepin Prevention Services Program Pilot in ClientTrack.
A referral allows users to connect clients with service providers for additional support.
Please note, A referral for services is different from a PATH Referral or a Coordinated Entry Referral.

A referral is a documented request to another agency or provider to assist a client with specific services. The process ensures that clients receive the appropriate resources based on their needs and eligibility.
To create a referral in ClientTrack, users must input essential client details, select the appropriate provider, and track the referral status.

Step-by-Step: Creating a Referral in ClientTrack
- From the Clients Workspace (1), create enrollment using Hennepin Shelter Hotline workflow (2).
2. Complete the intake forms for all clients in the Hennepin Shelter Hotline workflow.
- On the “Would you like to make a referral?” screen, select Yes.
- On the Referral Eligibility screen, select the Service Referral: “HSH-Homeless Prevention Services Program Pilot” (4), then select, Continue (5).
5. On the Referral screen, in the Refer to Provider field select, the magnifying glass (6) to search for Avivo- Hennepin County.
6. On the Find provider pop-up search screen.
i. In the Provider field: search Avivo.
ii. Select and select search.
iii. Select the provider Avivo-Hennepin County.
- Complete the Referral Source section:
i. Refer from Provider: Should automatically be filled in with the Organization selected in your User Settings.
ii. Status: Select "Referral Made".
iii. Enrollment: Ensure that the referral is linked to the client’s matching Project Enrollment: CCSPM-HP-HCC-DHS-CED-Hennepin County Diversion.
- Click the Next button at the bottom right of the screen.
- On the Voucher and Information Release screen:
i. Check the Email Authorized. This sends an email to Avivo users on the referral.
- Select, Next and on the Referral Outcome screen, select Finish.
- An email message pop-up will appear with the email address for Avivo users with the message: “This email is to inform you that a referral to your organization has been sent for Client:…….”. Then select, Send.

Step-by-Step: Creating a Referral in ClientTrack
- Once you have finished enrolling the client in your organization's project, continue to the next step.
- From the Clients workspace, in the menu group, select Referrals under the Enrollments and Services folder:
- On the Client Referrals form, click the Add New Referral button:
- Complete all required fields on the Referral section:
- Referral Service: Select a service.
For Avivo-RRH-HCC-Other-Homeless Prevention Services Program Pilot project, select one of the services under the Hennepin Homeless Prevention Services Program Pilot.
- Complete the Referral Recipient section:
- Refer to Provider: Avivo - Hennepin County
- Complete the Referral Source section:
- Refer from Provider: Should automatically be filled in with the Organization selected in your User Settings.
- Status: Select "Referral Made"
- Enrollment: Ensure that the referral is linked to the client’s matching Project Enrollment for accurate tracking. This is an important step even though the field is not marked as required.
- Click the Next button at the bottom right of the screen.
- Skip the Voucher and Information Release page by clicking Next.
- Skip the Referral Outcome pages by clicking Finish.
