How to Document a PATH Status

What this is
It is important for you to document a client's PATH Status as soon as it is determined. If not, services your client receives cannot be recorded and your client will not be included on PATH reports.
What you need to know
In HMIS, PATH Status is determined when your client:
- Meets the criteria for PATH Program eligibility (Homelessness + Serious Mental Illness)
- Has agreed to receive PATH Program services
Follow These Steps
To record a PATH Status for your client, follow these steps.
- Select the Action Button next to the active PATH project.
- Select Edit Enrollment
From the Household section - locate the PATH Status columns by using the horizonal scroll bar
Household section and the three PATH columns: Status Determined, PATH enrollment, and reason not enrolled - Answer the question, Date PATH Status Determined
As applicable, answer the Client became enrolled in PATH and Reason not enrolled in PATH
Focus on data fields for Status Determined and enrolled YES/NO
- Click Save
Helpful Tips
If the client leaves the project without enrolling, you still need to:
- Fill out the PATH Status section to show they were not enrolled. Explain why.
- If you were working with the client on the date their PATH status was determined, you must also record a Current Living Situation (CLS) for that date. If you couldn’t find the client, you can use “Unable to Locate Client” as the reason they weren’t enrolled.
