How to record a client’s interest in housing case management

This article explains how to record a client’s interest in Housing-Focused Case Management for Hennepin County programs using the Shelter Placement assessment.
What you need to know
This "interest in housing case management" is only for Diversion, Shelter Screening, Emergency Shelter, Outreach, Drop-in, or Case Management Hennepin County users entering data into projects where residents can be referred for Housing Case Management.
Follow these steps
1. Open the client’s record.
2. In the left-hand menu, expand the Care Coordination folder and select, Shelter Placement.
- Open or add new Shelter Placement Assessment.
- If the client already has a Shelter Placement assessment, click the edit pencil ✏️.
- If not, click +Add New to create one.
4. Record Case Management Interest.
- Scroll down to Interested in receiving housing case management?
- Select "Yes".
Selecting yes is what adds the client to the Housing Case Management Interest List. You are welcome to complete more of the form if the additional information would support care coordination.
5. Save your assessment.
When you are done
The client is now added to the Housing-Focused Case Management Interest List. Hennepin County staff coordinate follow-up and referrals for all clients on this list.
