How to use the LTH/HPH Data Checker

What this is
Need a way to check whether the date your client moved into housing is in HMIS? Check out the LTH/HPH Checker!
The LTH/HPH Checker helps you see with just a few clicks if program specific data elements are accurate for clients your housing program. This includes projects funded with MN Housing Ending Long-Term Homelessness (MN HSG-LTH) or MN Housing Housing Trust Fund (MN HSG-HTF)
Where to find the LTH/HPH Checker
- This tool is located in the Home workspace
- My ClientTrack -> Data Checkers -> LTH/HPH Checker
Follow these steps
Select your filters
- Organization: Select your agency
- Program: Choose your LTH/HPH or HTF program
- Date Range (optional): Leave blank if you only want to see currently enrolled clients or enter a start and end date.
- Filter by User or Case Manager: You can filter to only see clients assigned to a specific case manager or enroller
- Show Only Records with Errors: Check this box if you only want to see clients with missing data
- Show Clients Names: check if you want client names to display, otherwise only client ID will display.
- Click Search
What you will see
For each head of household enrolled, you will see:
- Client ID
- Enrollment date
- LTH/HPH Funding
- Assessment Date
- Assessment Type
- Housing Cost
- Reason for Leaving
- Household Staying at Exit
- Primary Subsidy Source
- Subsidy Status at Exit
- Subsidy Ending Reason
What you can do from here
Fix the data right from here. If data is missing or looks incorrect, you can update it directly. Just select the action button to update LTH/HPH program specific data elements for that enrollment.
Open the client record. You can also go directly to the client record using the action button.
You can use the LTH/HPH checker regularly as a data quality tool for your projects. Missing or incorrect data elements misrepresent housing outcomes and cause reporting errors.
