How to create a coordinated entry housing transfer

This article explains how Priority List Managers (PLMs) can create and document a Coordinated Entry (CE) housing referral transfer in HMIS using the "Housing Transfers" form in the clients workspace.

What you need to know

Priority List Managers (PLMs) responsible for documenting Coordinated Entry (CE) housing referral transfer requests in HMIS.

Before creating a transfer request:

  • The household must already have a housing enrollment connected to Coordinated Entry.
  • PLMs can only create or manage transfer requests for projects within their assigned CoC.
  • Cross-CoC transfers require coordination between the originating and receiving CoC.
  • Some fields only appear when the Request Outcome is marked as Accepted.
  • The resulting enrollment must already exist before it can be linked in the transfer form.

Location of form

The housing transfers form is located the Priority List Management folder in the clients workspace.

Follow these steps

1.Go to the Coordinated Entry Housing Referral Transfer form in the client record.

2. Click on the +Add New button.

3. Complete the Request Details section

Fill out the following fields:

  • Request Date
  • Request Type
    • Internal (within a single CoC) - choose this for a transfer to a housing project in your CoC.
    • Cross-CoC Transfer - choose this for a transfer to a housing project outside of your CoC
  • Request Reason
    • Voluntary
    • Involuntary
    • Emergency transfer
    • Project/program closure
  • Request Notes

4. Select the Originating Enrollment

In the Originating Enrollment field, select the enrollment connected to the client’s original housing referral or housing project enrollment.

Make sure the correct originating enrollment is selected before continuing.

5. Complete the Request Outcome section

Select the request outcome:

    • Accepted
    • Declined

Then complete the Outcome Notes field.

Additional transfer fields will only appear when the Request Outcome is marked as Accepted.

6. Complete the additional transfer details (if accepted):

Fields:

    • Accepting CoC
    • Acceptance Date
    • Accepting Program
    • Resulting Enrollment
    • Transfer Notes

7. Save the completed transfer request form.

When you are done

Review the completed transfer request

Confirm that:

  • The correct originating enrollment was selected
  • The accepting program matches the receiving CoC
  • The resulting enrollment is linked correctly

The Coordinated Entry housing referral transfer request will be saved in the client record and available for review by Priority List Managers and Coordinated Entry staff involved in the transfer process.
