What to do if my client passes away

What this is

This article walks you through what to do in HMIS if a client or a household member passes away.

What you need to know

A new data element was introduced into Minnesota's HMIS on October 1, 2026 to record if a person passes away.

Follow these steps

  1. Exit the client from your project. Follow the instructions on How to create an exit article to end the enrollment.
    1. The exit date should be the date the person passed away.
    2. For the destination, choose Deceased. For the exit reason, choose Death. Click Save.
    3. Go through the remaining forms and questions in the workflow.

If this person was the head of household and the other family members are staying in your project, reach out to the HMIS Helpdesk for guidance.

  1. Record death data on the client profile.
    1. Go to Profile on the client dashboard
    2. Scroll down to the Basic Client Demographics section the click the box to the right of the words "Client is Deceased"
    3. Complete the Date of Death and Source of Death Information fields.
    4. Once you are done completing the fields click Save.
