How to Record a Successful Coordinated Entry Referral

What this is

Resolving a Coordinated Entry (CE) Referral in HMIS documents whether your client was enrolled in your housing program. A CE referral recorded as Successful means your client was enrolled in your housing program.

What you need to know

This article is for Housing Providers who are participating in their CoC's Coordinated Entry system.

There are two possible CE Referral results in HMIS, successful or unsuccessful. Either result depends on what what happens as you work with your referred client. A successful referral means your client was enrolled in your housing program.

If your client is enrolled in your program but never successfully housed, their referral is still Successful. However, you will need to complete an extra step, and return to the referral outcome fields to record the reason the client did not receive housing.

Follow these steps

Successful Referral: Client Enrolled in Housing Program

🚨Before proceeding: verify your client has an active enrollment for your housing program

  1. From your Client's Record, click Enrollment and Services folder and then CE Event:

  1. From the list of events, find your client's CE Referral to your program, then click Edit:

  1. Complete the following fields:
    1. Referral Result: Choose Successful referral: client accepted
    2. Result Date: The date the client was enrolled in your housing program
    3. Resulting Enrollment: Choose the project enrollment you just created for the client prior to updating the CE Event.

  1. Click Save when you are finished.

Client Enrolled in Housing Program but Not Housed

You will need to complete this final step if your client does not become housed during their enrollment in your program.

  1. From your Client's Record, click Enrollment and Services folder and then CE Event.
  2. From the list of events, find your client's CE Referral to your program, then click Edit.
  3. Complete only the If Not Housed, Reason field and save your changes.

Do not make any other edits to the CE event at this point! Do not change the Referral Result and Result Date should not change.
