How to Record an Unsuccessful Coordinated Entry Referral

What this is
Resolving a Coordinated Entry (CE) Referral in HMIS is what documents if your client was enrolled in your housing program and housed. You will record a CE Referral as unsuccessful if your referred client was not ultimately enrolled in your housing program.
What you need to know
This article is for Housing Providers who are participating in their CoC's Coordinated Entry system.
There are two possible CE Referral results in HMIS, unsuccessful or successful. Either result depends on what what happens as you work with your referred client. An unsuccessful referral means your client has not been enrolled in your housing program and will not be.
Follow these steps
Unsuccessful Referral: Client Not Enrolled in Housing Program
- From your Client's Record, click Enrollment and Services folder and then CE Event:
- From the list of events, find your client's CE Referral to your program, then click Edit:
- Complete the following fields:
- Referral Result: Choose the option that best reflects the outcome of the referral:
- Unsuccessful referral: client rejected
- Unsuccessful referral: provider rejected
- Result Date: Record the date it was determined that the referral was unsuccessful.
- If Not Housed, Reason: Select the most appropriate reason from the picklist menu.
- Referral Result: Choose the option that best reflects the outcome of the referral:
- Click Save when you are finished.
