How to enter City Where Resides

City Where Resides is a is a program-specific data element for both Family Homelessness Prevention and Assistance Program (FHPAP) and Homework Starts with Home (HSWH) programs. This article overviews how to enter City Where Resides in HMIS, and what to do if it is missing.

What you need to know

City Where Resides appears on the Universal Data Assessment (UDA) in HMIS. It is entered during:

• Program enrollment
• Program update assessment(s)
• Program exit

Follow these steps

1. Start a program enrollment
2. Once on the Universal Data Assessment, select the city where your client resides:

If City Where Resides is missing from your client's enrollment:

1. Go to your client's record

2. Click the action button next to their enrollment in your program

   

3. Choose Edit Project Entry Workflow

   1. 

4. Move through the workflow until you get to the Universal Data Assessment

   

5. Once on the Universal Data Assessment, select the city where your client resides:

   1. 

6. Click Save.
7. Move through the remaining workflow forms. Click Finish to finalize.
