This article explains what is required in HMIS for your PATH (Projects for Assistance in Transition from Homelessness) program(s) in HMIS.
What you need to know
These requirements are for all projects funded by Federal Projects for Assistance in Transition from Homelessness (PATH) grant.
Program Specific Data Elements
Below is a list of program-specific data elements for PATH (PSDE). These are collected in addition to the standard HUD and Minnesota universal data elements (UDE).
