What you need to know
The Care Coordination folder is available for organizations operating in Hennepin Continuum of Care (CoC). If your organization also works in another CoC, you will see this folder even if you are entering data for the other CoC.
What is Hennepin Care Coordination?
The Care Coordination tools in HMIS bring together information from many parts of the client record to show an up-to-date snapshot of someone’s housing situation, services, and care coordination needs. These tools help users quickly review or update critical information to connect a client with the right resources.
How to access Care Coordination information in HMIS
- Navigate to the client’s record.
- In the Client Workspace, look at the left-side menu.

- In the Favorites section, you’ll see the Care Coordination Review (Read Only) form.
- This review is a read-only summary of care coordination information. It is used to easily review a selection of information about a client’s needs critical information for coordinating services.
- This summary can be saved or printed as needed
Careful to ensure you are following data storage and privacy rules for anything saved or printed from HMIS.
