This article will show you how to report changes to your HMIS project to ICA.

What should I do if my project has changed or closed?

The HMIS Project Change Request Form can be used to let ICA know that your project has either closed, needs its bed inventory changed, or has a change in a funding source. You can submit a single form to request multiple changes by selecting the appropriate Type of Request.

To use the HMIS Project Change Request Form:

  1. Open the Active HMIS Projects tab on the ClientTrack Projects page
  2. Find your project on the list
  3. Click on your project's row to open
  4. Click the purple Submit a Request button

  1. On the form that appears, select which change(s) you want to submit.
  2. Please complete all of the fields that appear

Tips if you are submitting a project close request

  1. Did you run all required final reports and submit them to your funder?
    1. If not, do this now
  2. Do you have any open client records for this project?
    1. Make sure they are closed
  3. Find any clients who still have open enrollments or open Services, and make sure have an exit date for before the project’s last day of operation
