Overview

The Connection with SOAR data element determines whether a client has ever been linked to the SOAR program, which in turn helps people apply for SSI/SSDI benefits. SOAR stands for SSI/SSDI Outreach, Access, and Recovery.

This data element is required for PATH projects by SAMHSA for federal reporting.

This data element is part of the PATH Workflow. It can be filled out during a project entry, update, or exit.

When is Connection with SOAR Collected in HMIS?

  • You can record if your client has ever been connected with SOAR at any point during your time working with them, no matter when they were connected to SOAR or who helped them.
  • At minimum, you should ask your client if they've ever been connected with SOAR by the time you fill-out their PATH Date of Engagement in HMIS.
  • Then, you should ask them again when:
    • Their Date of PATH Enrollment is entered in HMIS
    • When you enter their PATH Project Exit in HMIS
