This article is an overview of the HYA outcomes recorded in HMIS.

What you need to know

The Homeless Youth Act (HYA) Outcomes Assessment tracks the services and supports youth need when first enrolled (baseline), the services they receive, and the progress they make while in your program (outcomes). Only projects receiving HYA funding need to record outcome data.

Additional Tips

Don’t update outcomes every time a youth makes progress.

Wait until the next reporting period to record all changes at once. Only one outcome assessment should be completed per report period.

Example: If a youth joined your project on January 12th, got their license on July 7th, and earned their GED on September 3rd, report both achievements in the single HYA assessment for the July–December period.
