How to create a Housing Needs assessment

What this is

This article will show you how and when to create a Housing Needs assessment, which is what places your client on your region's Priority List for permanent housing resources.

What you need to know

Your client must first have been enrolled, and then exited, from your org's Housing Problem Solving (HPS) program before they can receive a Housing Needs assessment.

Follow these steps

The Housing Needs assessment can be started two ways:

  • During the HPS program exit workflow
  • From the client's record dashboard, within 14 days of their exit from the HPS

If you are creating the Housing Needs assessment during your client's HPS exit workflow:

  • The last form of the workflow will ask, "Proceed with the Housing Needs Assessment?"

    • Click Yes to begin the Housing Needs assessment

If you are creating the Housing Needs assessment from your client's record dashboard:

This is only available to you if you are within 14 days of the client's exit from the Housing Problem Solving enrollment.

  1. On the client's Dashboard, in the Enrollments section, find the client's current enrollment in your HPS program.
  2. Select Create Housing Needs Enrollment:

  3. Continue forward through all the forms that are presented to you
  4. On the Triage Assessment, make sure to select Placed on Prioritization List for Prioritization Status. Check out this article for more information.

  5. You will keep going, responding to the questions presented to you across the set of forms for your CoC
  6. You can click the Default Last Assessment button on each form to see if previously entered data pulls into it. Then, you can simply review/update that existing data, as needed.

  7. Once you have completed all the information, you should click FINISH.

When you are done

To check your client is on the Priority List, go to the client record. You should see the Housing Needs enrollment you just completed.
