Housing Navigation Workflow Overview
🚨Please Note 🚨 Housing Navigators should always be in your CoC's Coordinated Entry Organization. This is different from your "home" organization. Click here to see a list of programs and their owning organizations.
I was referred a client for Housing Navigation. What's next?
- Work with your client to confirm they want Housing Navigation.
- If yes, make sure your client has a Housing Problem Solving (HPS) or Coordinated Entry (CE) enrollment. Follow these steps to create one.
- If they decline, no further steps are needed in HMIS.
- Create a case manager assignment for your client.
- Assign yourself as the case manager by following these instructions
- This step logs the start of your client's housing navigation process.
How do I record the direct services, case notes, and referrals I've provided for my client?
Services
Follow these instructions to start creating a service for your client
- Required: Before saving your service, attach on of these three grants to it:
- Southeast CoC Housing Navigation – High Priority
- Southeast CoC Housing Navigation – DV
- Southeast CoC Housing Navigation – Rural
Referrals
Did you refer a client to another service provider? This is how you record your referral.
- From your client's record, select referral
- Click Add New Referral
- Fill out referral date, referral service
- For refer to provider, select your CoC's housing navigation option
- Select your client's HPS or CE enrollment
Did your client report a referral outcome?
- Return to the referrals form
- Select Referral outcome
- Add Date and Result
- Save!
Case Notes
Follow these instructions to create a case note for your client. Do not record services using cases notes - you will not be able to attach a grant and this will cause reporting issues for your program.
Current living situation
You are encouraged to document your client's current living situation.
My client is ending Housing Navigation. What are my last steps?
Exit your client from their HPS or CE enrollment if:
- Your client found housing**
- Your client leaves due to: non-contact, exit to an institutional setting for 3+ months, client request, or your client moved outside of your CoC's area
**If your client found housing but would still like ongoing housing navigation support, wait up to 90 days before following the steps below:
- End the case manager assignment
- Log a reason why client is no longer receiving navigational service
