How do I run a HPH Eligibility Confirmation form

What is this

MN Housing LTH/HPH funded units are required to be filled by Coordinated Entry referrals. This is an overview of how to produce a HPH eligibility confirmation form for your client's file.

What you need to know

Your client must be enrolled in your LTH/HPH project before you are able to generate this form.

This report is located in the Client Workspace.

  1. Use the dropdown to select the client’s LTH/HPH project enrollment.
  2. Click the Report button located in the bottom-right corner.

  1. When the report is finished running, it can be found in the Report Workspace.
  2. Under My Saved Reports.
  3. Click on the blue date to open the report.

How to Save a Copy of the HPH Eligibility Confirmation


  1. To save a copy, click the Save icon.
  2. Select the PDF option.

How To Print the HPH Confirmation Report


  1. Select Print Icon.
  2. A Print pop up will appear, click Print.

Note: This form contains client names. Do not email this report or store it in an unsecure location.

Why can't I run this report for my client?

You might be in the wrong organization

HMIS users with 'active' rights to the organization that created the LTH/HPH enrollment can generate the report, as well as the organization that made the referral.

Your LTH/HPH project is not showing up in the picklist

Confirm that you've enrolled the clients in your LTH/HPH project.

Then confirm the CE Event field "Resulting Enrollment" lists the project as well.

If one or both are missing, then your project will not display in the report prompt picklist.
