What this is
This article shows you how to resolve or update a non-Coordinated Entry referral in HMIS.
What you need to know
If you need help with Coordinated Entry referrals, check out this article: How to acknowledge a coordinated entry referral.
Step 1: Acknowledge the Referral
If your project receives a referral (outside of Coordinated Entry), your first step is to acknowledge it. This tells the sending agency that you're working on it.
- On the Client Dashboard, open Enrollment and Services > Referrals.
- On the Client Referrals page, find the referral and click the edit icon.
- Click Referral Outcome.
- Find the field Date Referral Acknowledged and use the calendar icon to choose the date you received the referral.
- Click Finish
Now, you’re ready to contact the client.
Step 2: Resolve the Referral
Once you’ve met with the client and determined eligibility, record the outcome in HMIS.
- On the Outcome Information screen, fill out:
Appointment Date – when you met with the client.
Result Date – the date you decided on the outcome.
Result – choose one:
- Services Provided
- Information Only
- Rejected
- No Show
- Comments (optional) – use this field for any notes.
- Click Finish to save.
If the client is eligible, you can now enroll them in your program.
If the referral was sent by mistake or needs to be reassigned, talk to the agency that sent it before making any changes.
