How to change the date on an assessment

This article will show you steps it takes to change the date on an assessment in HMIS.

What you need to know

When creating an assessment (entry, update, annual, or exit), it is important that the date of that assessment and all the dates on the forms in the workflow match. Mismatched dates can cause data quality and reporting errors.

We recommend using data checkers  for identifying and fixing missing or invalid data.

Steps to follow

When creating entering data in HMIS, the system will automatically default to today's date. If you are back entering data, or updating old data, it is possible you may end up with forms that have mismatched dates. Thankfully, this is easy to correct.

How to update assessment dates

  1. Click the action button (three dots) next to the enrollment and choose the assessment you need to correct.

  1. Go through the whole assessment and make sure to update each form in the workflow with the correct date. Remember to click Save on each form after updating the date.

If you are changing the dates on an assessment, make sure you change the Master Assessment Date to avoid data quality errors. To do this, click on Change Assessment date on the Universal Data Assessment:

How to update dates for other data

  1. Go to the section in HMIS where the data lives. For example, if you need to change the date on a service you entered, go to Services.
  2. Find the form you need to change and click on the edit icon to the left.

  1. After you update the date, remember to click Save.

If you do not have the option to edit a form, it is possible you are you are in the wrong organization. Check that you are in the same organization that the data was created under and try again.
